- Document Thoroughly: Maintain detailed electronic patient records, tracking outcomes and service plans.
- Work side-by-side with providers and social service agencies to support holistic, patient-centered care.

Compensation
- Type: Full-Time (40 hrs/week) - Monday through Friday 8am-5pm Salary: $18.91-$19.84/hr DOE
