About the role
- As a Physical Therapist in Home Health, you will provide skilled rehab services tailored to each patient's needs.
Responsibilities
- Develop and implement personalized rehabilitation care plans
- Document visits using WellSky (Kinnser) EMR
- Collaborate with experienced nurses, case managers, and physicians
- You'll deliver skilled therapy services in home-based, assisted living, and community care settings, helping patients regain independence and improve their quality of life.
Requirements
- Licensed Physical Therapist (PT) - California (Required)
- CPR Certification (AHA approved)
- Minimum 1 year of clinical experience (Home Health preferred)
- Strong clinical, communication, and patient education skills
- Valid driver's license, reliable transportation, and auto insurance
- ✔ Flexible Per Diem / PRN, Part-Time, or Full-Time opportunities
- ✔ Competitive per-visit pay + mileage reimbursement
- ✔ Hands-on experience in home health and rehabilitation care
- ✔ Work with geriatric and senior care populations
- ✔ Independence in a mobile, community-focused role

Compensation
- $120k-$150k
