About the role

  • Job Description: Title: Licensed Practical Nurse Division: Senior Services Status: Non-Exempt Reports to: Clinical Leadership Position will start as PRN and will eventually convert to Full-Time.
  • Travel: Continuous, necessary on a daily basis.
  • Vision: Normal: Consistent with standard workflow.

Requirements

  • Degree in nursing field as a licensed practical nurse.
  • Must have a valid driver's license, automobile insurance and qualified to drive under the organization's motor vehicle check.
  • Must have good verbal and written communication, comprehension, computer and interpersonal skills.
  • Must have the ability to speak, read, write and understand English.
  • Must have a current LPN license in good standing in the state(s) in which practicing.
  • Mental: Must have good verbal and written communication, comprehension, computer and interpersonal skills.
  • Licensure: Must have a current LPN license in good standing in the state(s) in which practicing.

  • 12 months facility/agency experience in nursing as an LPN preferred.

Compensation

  • Completes all annual education requirements timely, including but not limited to Compliance, Privacy, Security and HIPAA training and exhibits behavior as set forth by the Code of Conduct in the performance of their duties.

Benefits

  • Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
  • Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines to the registered nurse, RN Case Manager and/or Director of Clinical Management.
  • Provides mentoring, coaching, and education to the direct care staff, under the supervision of the RN.
  • Participates in interdisciplinary team meetings to coordinate the care of the client and family; exchange information and problem solve; and receive staff support and education.
