About the role

  • We are searching for a Case Manager I for our Casa Canal Permanent Supportive Housing Program!
  • The expected case load is up to about 20 participants.

Responsibilities

  • Case Planning and Monitoring Conduct interviews and research to identify risks, stratify needs, and develop data-driven care plans.
  • Provide financial coaching-including budgeting, tax filing (EITC), and credit repair-to support long-term stability.
  • Maintain a caseload of 20 households, facilitating case conferences and acting as a liaison between service partners and participants.
  • Perform routine reassessments and proactively adjust care plans to improve outcomes in static or regressive situations.
  • Manage case closures and exits-both voluntary and involuntary-ensuring all agency procedures and adverse action protocols are followed.
  • Data and Documentation Keep all relevant service data up-to-date in HMIS system and maintain case files in accordance with applicable guidelines, completing data entry immediately or no later than within 24 hours of service interaction.

Requirements

  • A valid California driver license, reliable personal vehicle, current personal auto insurance as required by law.
  • 1+ years' experience working with low income and/or homeless populations.
  • AA degree or Bachelors degree in lieu of experience will be highly considered!
  • This is an exciting role to gain Case Manager experience with the unhoused population.
  • Establish participant-driven goals based on resource availability, consensus, and required level of care.

  • $25k-$30k
