About the role

We are searching for a Case Manager I for our Casa Canal Permanent Supportive Housing Program!
The expected case load is up to about 20 participants.

Case Planning and Monitoring Conduct interviews and research to identify risks, stratify needs, and develop data-driven care plans.
Provide financial coaching-including budgeting, tax filing (EITC), and credit repair-to support long-term stability.
Maintain a caseload of 20 households, facilitating case conferences and acting as a liaison between service partners and participants.
Perform routine reassessments and proactively adjust care plans to improve outcomes in static or regressive situations.
Manage case closures and exits-both voluntary and involuntary-ensuring all agency procedures and adverse action protocols are followed.
Data and Documentation Keep all relevant service data up-to-date in HMIS system and maintain case files in accordance with applicable guidelines, completing data entry immediately or no later than within 24 hours of service interaction.

A valid California driver license, reliable personal vehicle, current personal auto insurance as required by law.
1+ years' experience working with low income and/or homeless populations.
AA degree or Bachelors degree in lieu of experience will be highly considered!
This is an exciting role to gain Case Manager experience with the unhoused population.
Establish participant-driven goals based on resource availability, consensus, and required level of care.
