Responsibilities
- Support daily discharge planning by coordinating and communicating with care teams, identifying post ‑ acute options, and updating Estimated Discharge Date (EDD) and placement details.
- 4) Maintain effective relationships with post ‑ acute providers and ensure up ‑ to ‑ date resource lists for care teams.

Requirements
- Bachelor's degree
- The role facilitates post‑acute referrals, tracks referral progress, completes required documentation and regulatory notifications, and documents discharge activities to help prevent delays,
- 1) Initiate, coordinate, and track referrals and transfers as assigned while maintaining accurate documentation in required systems.
- 6) Assist with insurance coordination, prescription support, form preparation, collateral gathering, and notary needs as required.
- One to three years ' experience in a healthcare setting assisting with discharge planning, care coordination support, or a related health science role
- Basic Life Support (BLS) certification active or within 30 days of hire
Skills
- Excellent verbal and written communication, collaboration and organizational skills. Ability to multi-task and follow-up independently.
- Working knowledge of common office software products (Outlook, Excel, PowerPoint)
- Department-specific competencies and their measurements will be developed and maintained in the individual departments.
- The competencies will be maintained and attached to the departmental job description.
- Responsible managers will review competencies with position incumbents.
Compensation
- $23k-$42k

- 2) Coordinate post ‑ acute services such as rehab, Skilled Nursing Facilities (SNF) placement, Visting Nurse Association (VNA), Durable Medical Equipment (DME), and MassHealth forms.
- 5) Facilitate access to behavioral health, substance use treatment, housing supports, and community resources.
- This position may have a signing bonus available a member of the Recruitment Team will confirm eligibility during the interview process.
