Responsibilities

  • Partnering directly with Clinical Leads, this role owns the day-to-day operational support of the care team, driving program and Individual Clinician metrics and ensuring high-quality, standardized care delivery.
  • Own the operations, documentation, and continuous evaluation of day-to-day practice workflows. Ensure workflows are optimized to reduce administrative burden while meeting clinical standards.
  • Lead operational change management for the clinical teams, in collaboration with clinical leads. Translate practice-wide changes into clear, actionable guidance and ensure smooth rollouts of new tools, strategies, or team structures.
  • Drive accountability for the operational, program, and individual clinician metrics (e.g., Q2 Dyad Metrics, Individual Clinician [IC] Metrics).
  • Serve as the primary operational partner for clinical leaders, unblocking day-to-day operational hurdles and managing resource/support allocation, where applicable.
  • Partner closely with Outcomes Operations Leads to operationalize solutions when plan-specific deep dives highlight a need for workflow tweaks, coaching, or supplemental list management within a specific clinical function.

Compensation

  • The expected salary range offered for this role is $95,000 to $115,000.
  • Actual compensation may vary based on relevant experience, skills, competencies, and certifications.
