Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers in order to maintain proper care coordination and continuity of care.
Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.

Must have basic demonstrated technology skills, including operation of a mobile device.

Education and experience, preferred Previous experience in home health, hospice, or pediatrics is preferred.
