About the role
- This program serves high-risk patients through an AI-enabled interdisciplinary model combining virtual and home-based care.
- You will serve as the clinical cornerstone of the care team, providing excellent patient care at critical moments including initial visits welcoming them into the program, and transitions from hospital to home.
- As both clinician and leader, you'll set the clinical and cultural standard for your team while maintaining accountability for medical outcomes across your patient panel through performance data analysis and metric optimization.
Responsibilities

Skills
- Board Certified Geriatrician, Primary Care, Internal Medicine, or Family Medicine Physician with at least 2 years clinical experience.
- Experience providing clinical care in collaboration with interdisciplinary teams.
- Experience working with complex, frail, or vulnerable populations, including in Skilled Nursing Facility (SNF) setting.
- Experience with virtual care is a plus but not required.

Compensation
- $210,000- $315,000 / year
- The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting.
