About the role
- This position is responsible for integrated patient care coordination under
- the direction of the Integrate Care Team model.
- Optimized and pulling data to identify high risk patients and engage them in care.

Nice to have
- BSW or closely related degree.
- Certified MA or LPN preferred, but not
- 3 years preferred.

- All training requirements including Relias at the beginning of employment and annually thereafter.
