About the role

  • The Senior Post Acute Transition Specialist functions as a highly specialized strategist, exercising significant autonomy to spearhead post-acute care initiatives for a flagship Enterprise Practice and its affiliated health system.
  • This role serves as an executive-level Account Manager, architecting deep-seated partnerships with health system leadership and orchestrating the care continuum across the local healthcare ecosystem.
  • Extensive use of computers and keyboard.

Responsibilities

  • Develop and present sophisticated collateral that synthesizes claims data, clinical insights, and performance analytics into high-impact operational workflows.

Requirements

  • Advanced proficiency in architecting and delivering data-driven collateral to drive change management and influence strategic decision-making.
  • Superior public speaking and facilitation capabilities, with the ability to engage and influence diverse executive-level stakeholders.
  • Previous experience in high-level Account Management, driving strategic alignment and fostering deep-seated professional partnerships.
  • Occasional walking and lifting may be required.
