About the role
- Prime Home Health is seeking compassionate and skilled LVNs working on PRN basis to join our in-home health teams in the Northwest San Antonio and surrounding areas.
- We have many visits available and offer trip charges to offset any longer distances.
- YOU MUST LIVE IN THE SAN ANTONIO AREA, THIS IS NOT A REMOTE POSITION.
Responsibilities
- Care Coordination: Collaborate with the interdisciplinary team, patients and their families.
- Safety and Compliance: Adhere to infection control protocols, maintain a safe environment for patients and staff, and comply with all relevant laws, regulations, and organizational policies and procedures.

Requirements
- Proficiency in using computers and apps such as Kinnser/Wellsky EHR, and Slack for internal communication.
- Strong clinical skills and knowledge of nursing principles and practices.
- Valid LVN license issued by the State of Texas
- Minimum of one year of experience as an LVN in an Adult In-Home Health setting is required.
- Current CPR BLS certification
- Excellent communication and interpersonal skills to interact effectively with patients, families, and the interdisciplinary team.
- Compassion, empathy, and sensitivity to the physical, emotional, and spiritual needs of patients and their families.
- Ability to work independently and as part of a team, demonstrating flexibility and adaptability in a dynamic healthcare environment.
- Commitment to maintaining confidentiality and upholding ethical standards in patient care.
- Reliable transportation
