About the role

Summary: The primary responsibilities of the clinical pharmacist include providing comprehensive pharmaceutical care to all patients within Baystate Health.
The clinical pharmacist will maitain competency, training, and ongoing education of pharmaceutical knowledge, integrated technology, and practice model changes.
Such documentation may include Clinical Interventions, Safety Reporting Events, etc.

Maintains accurate documentation of all clinical interventions/clinical notes.
Performs all clinical duties assigned to pharmacist position which may include IV to PO dosing, Antimicrobial stewardship programs, etc.
In the event the pharmacist is unable to perform their daily duties, they are expected to appropriately hand-off the task as necessary.
2 ) Distributive Pharmacy Services: Maintains compliance with the departmental expectations of accurately reviewing, verifying, and dispensing all pharmaceuticals in compliance with the departmental policies and procedures including regulatory compliance measures.
Utilizes effective modes of communication to escalate decision support to leadership as deemed necessary.
6 ) Regulatory Compliance: Documents and manages controlled substances in accordance with all state and federal guidelines.
7 ) Communication and Engagement: Maintains compliance in utilizing the following modes of communication as deemed appropriate per the BH and Departmental Pharmacy policies and procedures: email, web paging, shift to shift communication logs, and verbal hand off.
Incumbent is expected to perform other similar and related duties as assigned.

Each clinical pharmacist is encouraged to identify potential career paths and complete the required competencies/training to reach such personal goals.
Understanding the avenues for additional decision support are required. (I.e. Supervisor on Call and Chain of Command).
Required Work Experience:
It is the expectation of the clinical pharmacist to complete and document the appropriate tasks required to maintain patient specific record keeping in an electronic database.

1 ) Possesses excellent interpersonal, verbal, and written communication skills.
2 ) Understands the principles of staff engagement.
3 ) Proficient in computer skills (MS Word, Excel, PowerPoint).

Note: The compensation range(s) in the table below represent the base salaries for all positions at a given grade across the health system.
Typically, a new hire can expect a starting salary somewhere in the lower part of the range.
Pay ranges are listed as hourly for non-exempt employees and based on assumed full time commitment for exempt employees.
As defined by the Pharmacy Leadership Team, the clinical pharmacist will be expected to meet all departmental specific goals set forth on an annual basis.

Effectively utilizes communication tools available to formulate or communicate an established plan of follow up with the requesting health care professional/patient when necessary.
