Requirements
- Must hold licensure/certification as required by the state of Utah and DEA number.
- Successful completion of National Certification as a Physician Assistant or Nurse Practitioner. Must hold licensure/certification as required by the state of Utah and DEA number.
- Hours: Monday-Friday 8 a.m.-5 p.m. typically working 4 days/week

- Orders, interprets, and evaluates appropriate diagnostic and laboratory tests to identify and assess patient's clinical problems and health care needs.

- Documents appropriately in patient's medical record in a timely manner.
