Requirements

  • Must hold licensure/certification as required by the state of Utah and DEA number.
  • Successful completion of National Certification as a Physician Assistant or Nurse Practitioner. Must hold licensure/certification as required by the state of Utah and DEA number.
  • Hours: Monday-Friday 8 a.m.-5 p.m. typically working 4 days/week
