About the role
- This job facilitates and monitors the referral workflow from the acute care hospital or ambulatory outpatient setting to home.
- The incumbent works directly with the Transitional Care Coordinator (TCC) to coordinate the patient's post-acute care needs.
- The incumbent works to complete their work in an effective, efficient, accurate, and timely manner to assure compliance with regulations.
Responsibilities
- Partner with the clinical team to identify and obtain necessary supportive clinical documentation for compliance, regulatory or insurance/billing.
- Equipment delivery - Follows established workflows to assure delivery via internal hospital or external partners.
- Freedom of Choice: Engages patient and/or family/support person to obtain choice for post-acute care as indicated by policy and documents according to established workflow. (5%)
Requirements
- Associate's Degree
- 1 year of Customer Service

- High school diploma/GED
- Clinical or medical background
- Data entry
- 1 year experience with Electronic Medical Records (EMR)
- Preferred
- 1-3 years' experience in the medical profession

- Pay Range Maximum:
- Base pay is determined by a variety of factors including a candidate's qualifications, experience, and expected contributions, as well as internal peer equity, market, and business considerations.
- The displayed salary range does not reflect any geographic differential Highmark may apply for certain locations based upon comparative markets.
