About the role
- Responsible for managing all aspects of the provision of patient social services including managing the members of the social work team.
- Assesses social, psychosocial and emotional factors related to patients' illness to determine ability to cope with death and the dying process.
- Assesses grief needs, suicide risk and interventions.
Responsibilities
- Collaborates with hospitals and community agencies to facilitate continuity of care as needed
- Performs other tasks as assigned.
- Maintain a stable posture and gait with hands free to perform anticipated or routine and emergent patient care.
