- Transition plans completed prior to client discharge and include the road map for recovery success once treatment is complete.
Responsibilities
- Develops an accurate, comprehensive transition plan prior to treatment completion and shares with client, support system and referral source.

Requirements
- Associate's degree required.
- Years of Experience
- Experience equivalency may be considered in place of education requirement.
- Two year's professional experience working directly with youth and families in a child welfare, school, childcare, criminal justice behavioral health or related setting preferred.
Nice to have
- Bachelor's degree in psychology, social work, or related field is preferred.

- Effectively communicate via written, verbal, in person and virtual methods.

- Analytical skills.
- Knowledge of community resources
- Completes assessments upon intake.
- Assist clients in overcoming barriers to recovery by coordinating/facilitating communication with various community systems.
