About the role
- As a Home Health Physical Therapist (PT), you'll provide skilled physical therapy services under physician orders to help patients regain mobility, restore function, reduce pain, and maximize independence in the comfort of their homes.
- The PT is responsible for accurate clinical documentation and maintaining compliance with Medicare, state, and agency regulations.
Responsibilities
- Perform comprehensive patient evaluations, including OASIS assessments, and develop individualized treatment plans.
- Deliver therapeutic interventions to relieve pain, improve mobility, restore function, and maximize independence.
- Collaborate with physicians, nurses, PTAs, and other interdisciplinary team members to ensure coordinated, high-quality care.

- Dedicated scheduling and patient admissions support
Requirements
- Minimum of one (1) year of PT experience in an acute care setting.
- Experience with OASIS documentation and Medicare regulations.
- Knowledge of current physical therapy best practices and standards of care.
- Ability to prioritize responsibilities while delivering compassionate, patient-centered PT care.
