About the role
- Entyre Care helps elders and peoples with disabilities stay and health at home with trusted caregivers.
- This is hands-on, in-the-field nursing with real autonomy.
- This role serves as the operational bridge between members, family caregivers, managed care organizations, and Entyre's internal teams to ensure a seamless and compliant care experience.
Responsibilities
- Act Like Owners: Take initiative and move fast to drive results for families.
- Leverage your expertise to support caregivers and ensure they have the resources and confidence to care for their loved ones at home. ·
- Develop individualized plans of care following each home visit, reflecting client preferences and physician direction
- Obtain and maintain signed physician orders; renew every three months or on a change in condition
Nice to have
- Active, unrestricted Wisconsin RN license (or multistate compact privilege)
- Experience in home care, personal care, or community-based nursing preferred
- Comfort working independently in clients' homes across Milwaukee County
- Valid driver's license and reliable transportation
- Strong documentation habits - accuracy here protects clients and the agency alike
