  • Essential Duties: Under the general supervision of the Branch Manager, the Hospital Care Coordinator is responsible for coordinating durable medical equipment (DME) and respiratory services.

Responsibilities

  • Maintain accurate records of referrals, orders, and communications.
  • Maintain inventory levels in assigned hospital convenience closets.
  • Collaborate with Customer Care and Dispatch teams as needed to ensure efficient service delivery.
  • Perform all other duties as assigned by department leadership.
  • Present all patients with their right to freely choose a DME supplier and document such choice prior to coordinating services.
  • Our goal is to be the most trusted and preferred resource to manage high-risk, post-acute respiratory patients through our high-touch clinical team and high-tech, innovative solutions.

Requirements

  • Ability to multitask and meet deadlines.
  • Ensure all required documentation and physician orders are received, complete, and accurate and patient eligibility is verified prior to engaging with patient or authorized family member/care giver.
