Responsibilities
- Deliver excellent customer service for every member including assisting with care coordination needs such as referrals, enrollments in chronic disease management programs, etc.
- Perform a high volume of outbound and inbound member calls daily to support scheduling, pre-visit preparation, and care coordination.
- Provide real-time telehealth support, including guiding members through device setup, troubleshooting connectivity issues, and ensuring successful provider visit completion.
- Support multiple providers simultaneously (typically 3-4), including managing schedules, completing pre-visit chart preparation, and coordinating care needs.
Requirements
- While performing the duties of this job, the employee is regularly required to talk or hear.
- The employee regularly is required to stand, walk, sit, use hand to finger, handle or feel objects, tools, or controls; and reach with hands and arms.
Nice to have
- Experience working with internal medicine offices
- LVN / LPN or other higher nursing certifications accepted as well.
Compensation
- $38,569.00 - $57,853.00 Pay range may be based on a number of factors including market location, education, responsibilities, experience, etc.
