About the role
- This is a high-priority leadership position for a qualified Registered Nurse (RN) focused on coordinating, leading, and supporting patient care services within a structured home health setting.
- Operating in a hybrid capacity out of San Antonio, TX, you will take complete ownership of caseload planning, referral reviews, clinical order approvals, and clinical staff alignment while strictly adhering to compliance and regulatory standards.
Responsibilities
- Interdisciplinary Communication: Facilitate case conferences, maintain communication with physicians, schedulers, therapists, and field staff to ensure continuity of care.
- Supervisory Support: Support the Director of Nursing by supervising field staff, assisting with scheduling, and participating in call rotation/triage responsibilities.

Requirements
- Software Proficiency: Hands-on proficiency with Homecare Homebase (HCHB) software systems.

Compensation
- $105,000 - $142,000 (Total Compensation Range) + Up to $32,000 Annual Bonus
