- ROLE As a Part-Time (1099) Remote Nurse at Sensorum Health, you will leverage your clinical expertise, particularly in emergency care, to identify patients experiencing clinical decompensation.
Responsibilities
- Provide clinical decision-making support and recommendations for next steps in care.
- Collaborate with Sensorum's partners and operations teams to ensure timely follow-up and care delivery.
- Document all assessments, clinical findings, and care recommendations accurately in the system.
- You will monitor and interpret patient data, conduct telephonic assessments, and manage care coordination to ensure timely interventions.
Nice to have
- Experience managing older adult or high-risk patient populations.
- Background in chronic disease management and care coordination.
- Prior experience working in telehealth or startup environments.
