About the role
- Pay Range: $35.92 - $53.88 A successful candidate's actual pay rate will be based on several factors including relevant experience, skills, training, certifications and education.
- Hospital Sisters Health System (HSHS) is seeking a Care Manager-RN to join our NICU team.
- In collaboration with the patient/family, social workers, physicians, interdisciplinary team, and payer representative, the Care Manager-RN ensures patient progression through the continuum of care in an efficient and cost effective manner.
Responsibilities
- Identifies, initiates and manages optimal care coordination and discharge planning to develop patient flow/throughput to enhance continuity of care, smooth and safe transitions, patient satisfaction, patient safety and length of stay management.
- Provides support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs.
- Adoption Assistance provides financial support up to $7,500 for colleagues growing their families through adoption to reimburse application and legal fees, transportation, and more!
Nice to have
- Associate's Degree or Diploma in Nursing is required.
- Bachelor's Degree in Nursing is preferred.
- 2 years of clinical experience is required.
- Prior case management experience is preferred.
- Quality/Process Improvement experience is preferred.
- License as a Registered Nurse (RN) in the state of practice is required.
- Case Management certification (ACM, CCM, RN-BC) is preferred.
- Basic Life Support Certification (BLS) is required within 60 days of hire.
Compensation
