About the role

We are seeking a compassionate, organized, and relationship-driven Care Coordinator to support individuals with developmental disabilities and the caregivers who serve them.
  • As a Care Coordinator, you will play a vital role in ensuring members and families receive consistent, high-quality support while helping caregivers feel connected, supported, and successful. You will serve as the primary point of contact after services begin, helping maintain service stability, resolve concerns, coordinate care, and strengthen

Responsibilities

  • Build strong relationships with members and their support teams.
  • Ensure services align with member goals, needs, and authorized support.
  • Develop and maintain positive relationships with assigned caregivers.
  • Support long-term service stability and positive outcomes for both members and caregivers.
  • Support caregiver recruiting, onboarding, and member intake activities as needed.
  • As a Care Coordinator, you will play a vital role in ensuring members and families receive consistent, high-quality support while helping caregivers feel connected, supported, and successful.
  • You will serve as the primary point of contact after services begin, helping maintain service stability, resolve concerns, coordinate care, and strengthen member-caregiver relationships.

Requirements

  • Must be at least 18 years old.
  • Bilingual in English and Spanish required

Compensation

  • $19k-$22k
