About the role
- Acts as liaison between PT and community employers; is responsible for growth and development of these services/programs.
- Acts a liaison between staff and physicians meeting the need of the customer.
- Assures continuation of therapeutic plan following discharge by designing home exercise programs; instructing patients, recommending and/or providing assistive equipment; recommending follow-up programs.
Responsibilities
- Maintain patient records confidentially.
- Maintain professional appearance and personal conduct at all times.
- Establish and maintain effective working relationships with physicians, staff and management.
- Promptly and thoroughly documents and maintains clinical and administrative records regarding patient limitations/interventions, treatments, responses and progress to administered therapy as well as discharge summaries.
