About the role
- The Chronic Care Management program aims to facilitate high-value, patient-centered care that improves timely access to and provision of preventive services and chronic disease treatment.

Responsibilities
- Collaborate with primary care teams to manage chronic disease patients using the Phamily platform.
- Engage in quality improvement efforts and support care redesign strategies.
- Perform all job functions in compliance with applicable federal, state, local, and company policies and procedures.
- Manage a caseload of 500 patients, with 300 billable by the end of each month.
- Provide data to the care teams to properly perform these processes.
- Each Care Manager will be expected to manage a 500-patient caseload with 300 billable by the end of the month.
Requirements
- Must be ambulatory on stairs in the event of an emergency.
- Requires ability to work in a high-stress environment.

- LICENSURE OR CERTIFICATION: CMA preferred
