About the role

  • The Bureau of Equitable Health Systems (BEHS) is the healthcare bureau of DOHMH.
  • Experience serving as a practice facilitator, quality improvement advisor, technical assistance provider, or program manager supporting primary care practices.
  • Knowledge of NCQA Patient-Centered Medical Home standards, quality measurement, clinical workflow redesign, care coordination, population health management, and performance improvement methodologies.

Responsibilities

  • Develop systems to track and analyze demographics, trends, and external activities with large primary care practices.
  • Lead strategic discussions and contribute to the development of new programs and funding applications.
  • Partner with the Evaluation team to assess program impact and effectiveness, disseminating findings through reports and presentations.

Skills

  • Demonstrated experience supporting Patient-Centered Medical Home (PCMH) transformation, recognition, or renewal activities.
  • Familiarity with quality improvement methodologies such as Plan-Do-Study-Act (PDSA), Lean, or Six Sigma.
  • Possession of a Patient-Centered Medical Home Certified Content Expert (PCMH CCE) credential is strongly preferred.
  • Proficiency in Microsoft Office Suite; experience with CRM systems (e.g., Salesforce) preferred
