  • Job Title: Aging Life Care Professional Location: Lower Fairfield County Job Type: Full-Time This role is ideal for a professional with deep experience in aging and care management, who is passionate about advocating for older adults.

Responsibilities

  • Creates goal-oriented care plans to address concerns.
  • Responds quickly to emergencies and high-risk situations Presents recommendations to the client and care team, involving all relevant parties in decision-making Implements and maintains the care plan, addressing obstacles and updating goals as needed.
  • Performs periodic reassessments to ensure the care plan meets goals and adjusts based on feedback Coordinates care, arranges services, and makes referrals for clients.
  • Whether offering occasional consultations or urgent crisis support, we tailor our involvement to meet each individual's unique needs.

Requirements

  • 3-5 years' experience working with older adults and families.
  • knowledge of family dynamics and family systems.
  • Knowledge and comfort using electronics including a "smart phone", laptop/desktop computer.
  • Obtain and maintain certification as required by ALCA.
  • Actual compensation may vary based on factors such as licensure, years of experience, and other relevant qualifications.

Compensation

  • $75k-$80k
