Responsibilities
- The Discharge Specialist will work collaboratively with Consumers to develop comprehensive discharge plans for every Consumer in the Behavioral Care Center (BCC).
- Meets one-on-one with BCC C's to collaboratively develop a discharge plan that addresses that

Requirements
- Ensures timely and appropriate documentation as required by the Agency.
- Ability to prioritize, remain calm and focused when dealing with consumer crisis
Skills
- Bachelor's degree in health-related field of counseling, psychology, or social work
