About the role

  • The Case Manager plays a critical role in supporting client continuity of care by coordinating discharge planning, facilitating Medicaid enrollment, scheduling follow-up appointments, and ensuring seamless transitions between levels of care.

Compensation

  • Full-time | Salaried | Exempt The pay range for this position is $50,000 - $65,000 annually, set in good faith based on the responsibilities of the role, qualifications, and market conditions.
