About the role
- Community Health Care Workers (CHWs) work in the neighborhoods where patients reside connecting and linking patients and their families to resources and the care they need.
- CHWs will complete CHW Training and Certification after hire.
- Establishes peer-to-peer relationship in order to function as a link between the community and the health system.
Responsibilities
- Accountabilities Develops strategic partnerships in the community to support community outreach and referrals.
- Provides guidance and support for patients through home visits and support of care coordination programs.
Requirements
- High School diploma or equivalent OR post-high school diploma / highest degree earned 1 year of community involvement experience.
