About the role

  • This role will receive referrals for individuals from at-risk populations from interdisciplinary team members (including physicians, Case Managers, staff nurses and other members of the care team).
  • Must demonstrate knowledge of the principles of growth and development as it relates to the different life cycles.

Requirements

  • Master's Degree (Required)DL-HC - Driver's License only if in a Home Care department - Department of Motor Vehicles, LSW - Licensed Social Worker - Social Work Certification and Licensure Board
  • Masters degree in Social Work or a related field and LSW license in the State of Ohio.
