About the role
- This position offers the opportunity to work collaboratively with a multidisciplinary team focused on delivering high-quality, patient-centered care for individuals with serious and complex illnesses.
- The APP will provide inpatient consultation services while contributing to education, academic initiatives, and program development.
- Assist patients and families with symptom management, goals-of-care discussions, advance care planning, and end-of-life care needs.
Responsibilities
- Collaborate with physicians, APPs, nursing staff, and interdisciplinary team members to develop and implement patient-centered treatment plans.
Nice to have
- Preferred Previous palliative care, hospice, critical care, or ICU experience.
- Completion of a Palliative Care Fellowship Program.
- Certification in hospice and palliative care, including:
- Advanced Certified Hospice and Palliative Care Nurse (ACHPN), or NCCPA Certificate of Added Qualifications (CAQ) in Hospice and Palliative Medicine.
- Candidates without certification are expected to obtain ACHPN or CAQ certification within two years of employment.

Compensation
- RVU incentive opportunity.
- Rotating remote night and weekend call, approximately 7 weeks per year Key Responsibilities Provide comprehensive inpatient palliative care consultations and follow-up care.
