About the role

  • Hours will fall Monday - Friday, day shift
  • This RN will work with a group of physicians, fellows, and nurse practitioners to provide care coordination for their patients.
  • The RN will communicate with patients, families, and ancillary services through Epic telephone encounters/My Chart encounters and via phone.

Responsibilities

  • Plans, implements, educates and evaluates care using Evidence-Based Practices.
  • Documents according to organizational standards, policies, and procedures.

Requirements

  • OCCASIONALLY: Biohazard waste, Climb stairs/ladder, Driving motor vehicles (work required) *additional testing may be required,
  • BSN or Associate degree with commitment to obtain BSN within 5 years of hire, required.
  • Current CPR certification required.
  • Must be willing to obtain and maintain NRP, PALS, ACLS, APHON or other necessary certification as determined by your manager based on business unit.
  • Certifications: Current CPR certification required.
