About the role
- The position promotes continuity and effective transitions for clients and families and addresses their ongoing needs.
- The role involves collaborative leadership, formal education sessions, and coaching to enhance the provision of quality care.
- Service Provide direct clinical assessments, consultations and primary care within the RN extended class scope of practice for clients with substance use, mental health issues and gambling concerns.
Responsibilities
- Team Lead provides non-urgent primary care assessments in person or by telemedicine to clients.
- Support the dissemination of research, knowledge and innovative practices.
