Requirements
- John's Hospital, also supports Litchfield, Decatur, Shelbyville, and Effingham, Travel will be required

Nice to have
- Bachelor's degree in Nursing or Social Work is required.
- Master's degree in Nursing or Social Work is preferred.
- In lieu of bachelor's degree diploma or an associate's degree in nursing or in relevant clinical field with a minimum of 3 years of case management experience may be considered.
- 3 years leadership experience in a progressively responsible role is required.
- 2 years of case management experience is preferred.
- Certifications Licenses and Registrations
- Licensed Registered Nurse or Social Worker in the state of practice is required (IL or WI).Case Management certification (ACM CCM RN-BC) is preferred.
- In lieu of licensure in Nursing or Social Work may consider candidates with other clinical credentials.
Compensation
- $45.84 - $68.76 A successful candidate's actual pay rate will be based on several factors including relevant experience, skills, training, certifications and education.
