About the role

Wider Circle is hiring an Associate Care Advocate to join our Care Delivery team.
This is an entry-level job with a clear path to move into a more senior role.
In this job, you will call patients by phone to learn what challenges affect their health.

Manage Your Schedule You will keep your own daily calendar, send reminder calls to reduce missed appointments, and meet weekly goals.
Able to manage your own tasks, stay on top of notes, and meet goals without close supervision.
Self-Directed: Able to manage your own tasks, stay on top of notes, and meet goals without close supervision.
You build trust quickly, listen well, and communicate clearly.

You must be able to sit, stand, and walk as needed.
You must be able to type and use multiple computer programs at once while talking.
A high school diploma or GED is required.
If you do not have one, you must be willing to earn it by attending training provided by Wider Circle and/or completing a state certification application.

Speaking both English and Spanish (or another language) is strongly preferred.

Comfortable using websites and apps to enter and track data.
Able to adapt in a fast-moving work environment.
Background Check: Willing to complete a drug test and background check before starting.
Tech Skills: Comfortable using websites and apps to enter and track data.

These may include things like trouble getting food, finding housing, or getting to medical appointments.
You will do well in this role if care about helping others and want to reduce health inequity.
About Wider Circle Wider Circle works with health plans and doctors across the country to run community care programs.
We connect health plan members with neighbors who share similar experiences.
Together, they support and encourage each other to take better care of their health.
Screen Patients and Manage Intake You will call patients and ask health-related questions using a set script.
Then you will share resources and send referrals to health and mental health services.
Keep Accurate Records During each call, you will type patient responses into a health records system called an Electronic Health Record (EHR).
You will also schedule follow-up calls for patients and leave time between appointments to finish your notes.
