About the role
- As a Physical Therapist (PT), you'll provide skilled home health therapy services under the direction of the attending physician.
Responsibilities
- Develop, implement, and modify individualized plans of care.
- Collaborate with physicians and the interdisciplinary care team to ensure quality patient outcomes.

Requirements
- Minimum of 1 year of PT experience in an acute care setting required.
- Ability to work independently while managing daily schedules.

- Complete accurate clinical documentation within required company and Medicare timeframes.
- Participate in case conferences, on-call rotation, required trainings, and agency meetings.
- Current Physical Therapist (PT) license in the state of employment.
- Home Health experience preferred.
- Strong clinical judgment and problem-solving skills.
- Excellent communication and interpersonal skills.
- Valid driver's license, reliable transportation, and current auto insurance.
- Physical Requirements
- This position requires frequent standing, walking, bending, reaching, lifting up to 50 pounds (with or without assistance), driving between patient homes, and completing clinical documentation.
