About the role
- Phamily is assisting The NeuroMedical Center in hiring a Chronic Care Coordinator to work on-site using the Phamily Chronic Care Management platform.
- This role focuses on gathering and organizing patient data to identify unmet needs, engage patients in their care, and improve communication between patients and their care teams.
- Each Care Manager is expected to handle a 500-patient caseload with 300 billable patients by the end of the month.
Responsibilities
- Engage patients in their own care and support treatment interventions
- Maintain ongoing communication with patients and their care teams
Requirements
- Minimum of 2 years of experience in an outpatient clinic setting as a patient caregiver, educator, or advocate
- Ability to work independently and adapt to a fast-paced environment
- This role requires standing and walking for extended periods, occasional lifting of up to 50 pounds, manual dexterity, and the ability to handle high-stress situations.
Compensation
