About the role

  • The Clinical Pharmacist also participates in formulary management, medication-use evaluations, quality improvement initiatives, and staff/patient education.
  • Conduct comprehensive medication therapy reviews for patients on specialty medications, ensuring safety, efficacy, and adherence.
  • Provide disease-specific counseling and education to enhance patient understanding and engagement in therapy.

Responsibilities

  • Partner with prescribers, nurses, case managers, and payors to facilitate initiation and continuity of specialty therapies.
  • Document and report interventions and outcomes to support quality improvement and compliance.
  • Maintain documentation and reporting to support audits and accreditation readiness.
  • Participate in developing clinical protocols, therapeutic management programs, and formulary strategies.
  • Lead or support initiatives that optimize workflow efficiency, patient satisfaction, and overall program growth.

Requirements

  • A PharmD or bachelor's degree from an accredited College of Pharmacy is required.
  • Completion of PGY-1 Pharmacy Residency or 3+ years working as a clinical pharmacist in direct patient care is required.
  • An active Pharmacist licensure (in good standing) to practice pharmacy in the state in which you are applying in or and/or the ability to acquire this at the time of hire is required.
  • Must possess or be able to obtain a State Board of Pharmacy Preceptor's license by the current state you are applying to within one year of hire.

Nice to have

  • 1+ years of work experience in ambulatory care pharmacy setting post PGY1 training or 3+ years post PharmD is preferred.
  • Completion of a PGY-2 Ambulatory Care or Specialty Pharmacy Residency/Fellowship) is preferred.

Skills

  • Demonstrated expertise in medications, drug interactions, medical procedures, therapies, diagnoses, and disease states is essential.
  • Organized work approach with a proficiency in maintaining accurate documentation.
  • Strong verbal and written communication skills; proficiency in additional languages is advantageous.
  • Self-directed with strong self-management abilities; capable of identifying and addressing issues independently.
  • Proven track record in process improvement and a keen eye for detail and precision.
  • Proficiency in Microsoft Office programs (e.g., Word, Excel, Outlook) and web browsers.
  • Completion of an ASHP accredited PGY-2 Ambulatory Care Pharmacy residency (required); and BCACP eligible or certified (required);
  • New York State pharmacy licensure/license eligibility with eligibility for CDTM and Immunization endorsements (required); and

Compensation

  • The current annual compensation range for this position is $126,500 - $158,000.
