About the role
- This position is ideal for an RN seeking a PRN, Part-Time, or On-Call opportunity providing in-home hospice care.

Responsibilities
- Perform comprehensive assessments, including pain and symptom management, psychosocial support, and ongoing evaluation of patient needs
- Develop and implement individualized care plans in collaboration with the interdisciplinary team, patients, and families
- Provide emotional support and guidance to patients and families throughout the hospice journey
- Collaborate closely with physicians, social workers, chaplains, aides, and other team members to ensure continuity of care
- Maintain accurate and timely clinical documentation in compliance with hospice regulations and agency standards
- Participate in bereavement support and family follow-up as appropriate
Requirements
- Minimum of 2 years of hospice nursing experience required
- Experience with pain management, symptom control, and end-of-life care
- Reliable transportation and valid driver's license required
- The ideal candidate must have a minimum of two years of hospice experience, be bilingual in English and Spanish, and reside in the San Antonio area.
Nice to have
- Bilingual English/Spanish preferred
- Knowledge of Medicare Conditions of Participation and Joint Commission standards preferred
