Provide hands-on support with daily community integration and life skills activities.
Develop and maintain relationships with local organizations, businesses, and community partners to create inclusive experiences.
Maintain accurate documentation of care and community engagement activities.

This position requires extensive driving.
Reliable transportation and a valid driver's license are essential.
This role is highly mobile and community-based - candidates must be comfortable spending the majority of their time driving and supporting individuals out in the community, rather than in an office or facility.
Identify and coordinate opportunities that promote social connection and community inclusion based on individual interests.
Follow the Plan of Care developed by the Delegating Nurse and provide feedback on client progress.
Prioritize and respond to client needs with professionalism and empathy.
Ensure client safety and well-being in all settings.
Listen actively to client preferences and advocate for their inclusion in community life.
