About the role
- Join our Primary Care Health Home team at Centerstone as a Care Coordinator, where you'll play a key role in helping ensure clients receive coordinated, high-quality care.
Responsibilities
- Coordinate and distribute reports to the HCH team, Community Support Specialists (CSS), and community providers as needed.
- Provide technical assistance and training support to HCH staff and Community Support Specialists.
- Support quality assurance initiatives and comply with utilization review procedures and clinical records requirements.
- Ability to manage multiple priorities while maintaining attention to detail.
Requirements
- Proficiency with Microsoft Office applications.
- Track client activity and monitor required screenings, assessments, and reporting measures to ensure compliance and timely submission.
- Attend required state meetings, trainings, and program updates.

- Experience working with data tracking, reporting, or compliance documentation.

- Provide administrative and clerical support to the Healthcare Homes (HCH) Director and team.
- Prepare and compile monthly reports for review by HCH leadership prior to submission to the Missouri Department of Mental Health (DMH).
- Maintain monthly logs and documentation related to Healthcare Homes program activities.
