Requirements
- Ability to hear in normal range and wear a headset / earpiece
Skills
- New York LPN licensure is required.
- Maintaining a License is an ongoing requirement of this role
- Minimum of 2-years work experience in a healthcare setting involving patients with complex chronic disease states preferred.
- Cronic Care Managment experience preffered.
- Proficiency with a personal computer
- Technical and analytical skill set
- Excellent communication, organizational, and interpersonal skills are necessary
- Experience with Microsoft Excel preferred
- Strong communication, organization, and computer skills are essential.
Compensation
- The target salary range for this role is $20 - 26 per hour.
- An individual's salary within this range is based on multiple factors including but not limited to skills, experiences, licensure, certifications, and other business and organizational considerations.
