About the role
- Join a compassionate interdisciplinary home health team providing skilled physical therapy (PT) services to patients in their homes.
Responsibilities
- Maintain timely, accurate documentation and regulatory compliance
- We CARE for our patients like they are our own FAMILY.
- You'll evaluate, develop, and implement individualized treatment plans to help patients restore function, improve mobility, and reduce hospital readmissions while supporting patients and families throughout their recovery journey.
- Dedicated support team for scheduling and admissions
Requirements
- Reliable transportation and ability to travel throughout assigned territory
- Ability to lift up to 50 lbs. with or without assistance
- Minimum 1 year of PT experience in an acute care setting
