About the role
- This critical role serves as the primary liaison between hospitals, physicians, discharge planners, referral sources, patients, and clinical teams to ensure a seamless transition of care.
- The ideal candidate will have a strong clinical understanding of home health and hospice services and experience coordinating patient admissions in a fast-paced healthcare environment.
- Candidates with significant hospital, hospice, home health, or healthcare intake experience are encouraged to apply.
Responsibilities
- Ability to prioritize multiple referrals and manage a high-volume workload.
Requirements
- Preference will be given to candidates who are licensed Registered Nurses (RN) or Licensed Practical Nurses (LPN); however, licensure is not required.
- Current RN or LPN license (preferred but not required).

- Experience in Home Health and/or Hospice Intake.
- Minimum 5-7 years of healthcare intake, admissions, care coordination, discharge planning, or referral management experience.
- Experience working in a hospital, hospice, home health, skilled nursing, rehabilitation, or healthcare setting.
- Strong knowledge of medical terminology, patient care coordination, insurance verification, and healthcare documentation.
