- Position Overview United Health Services (UHS) is seeking an Ambulatory Clinical Pharmacist to provide comprehensive medication therapy management and disease state management services within the ambulatory care setting.

Responsibilities
- Manage patients with chronic conditions including, but not limited to, diabetes, hypertension, hyperlipidemia, heart failure, asthma, COPD, anticoagulation therapy, pain management, smoking cessation, and geriatric medication optimization/deprescribing.
- Utilize evidence-based guidelines and clinical expertise to assess, monitor, and optimize medication regimens to improve patient outcomes and minimize medication-related risks.
- Collect, interpret, and evaluate relevant clinical, laboratory, diagnostic, and imaging data to support medication-related decision making.
- Recommend and implement clinically appropriate treatment modifications under collaborative practice agreements and organizational protocols.
- Educate patients and caregivers on medications, disease states, treatment plans, self-management strategies, and adherence techniques to support active participation in care.
- Program Development & Operational Support
- Collaborate with physicians, nursing, clinic operations, and administrative leadership to integrate pharmacy services into clinical workflows and enhance patient care delivery.
- Ability to assess, monitor, and optimize medication therapy to improve patient outcomes and medication safety.
Requirements
- Doctor of Pharmacy (Pharm.D.) degree from an accredited college or university.
Compensation
- per hour, depending on experience
