Responsibilities

  • Develop and implement patient care plans across the domains of diet, exercise, sleep, and stress management
  • Collaborate with prescribing providers on medication choices

  • We're looking for an experienced RD/CDCES or RN/CDCES - referred to as a Diabetes Care Specialist at Ryse - to support patients virtually and in-person at our current and future clinics.

  • Coordinate care with Ryse prescribing providers, behavioral health specialists, exercise specialists, and health coaches
