Responsibilities
- Develop and implement patient care plans across the domains of diet, exercise, sleep, and stress management
- Collaborate with prescribing providers on medication choices

- We're looking for an experienced RD/CDCES or RN/CDCES - referred to as a Diabetes Care Specialist at Ryse - to support patients virtually and in-person at our current and future clinics.

- You'll build deep relationships with our patients and support them on their journey to better health while working closely with a top-tier team.
- Support patients in better health - measured by blood sugar, blood pressure, and lipids - at lower cost
- Empower patients to reach their health goals by using motivational interviewing techniques and continuous glucose monitor (CGM) data
- Coordinate care with Ryse prescribing providers, behavioral health specialists, exercise specialists, and health coaches
- Other activities to support patients to better cardiometabolic health in the context of team-based care
