About the role
- Provide transportation to individuals as needed.
- Assist with medical care (medication administration, and transportation to medical appointments).
- Facilitate and monitor activities consistent with Individual Service Plan goals and objectives for assigned caseload.
Responsibilities
- Participate with the team in development and implementation of the Individual Service and Support Plan.
- Coordinate other resources as needed, including support in arranging and/or directly providing transportation for participants.

Skills
- High School Diploma or Equivalent required.
- Experience working with the IDD community or other at-risk population
- Experience working with Electronic Health Records (EHR)
- Valid Driver's license
- Proficiency in computer skills including Microsoft Office and Electronic Charting Systems.
- Must be able to pass a background check, drug screen, and obtain DOT certification.
Compensation
- $20k-$24k
