About the role
- As a vital member of our interdisciplinary team, you'll work under the supervision of a Clinical Manager to coordinate all aspects of patient care-ensuring individualized, compassionate support that maximizes quality of life.
Responsibilities
- Develop and manage comprehensive care plans in coordination with the care team
- Collaborate with physicians, hospice staff, volunteers, and external agencies to ensure optimal support

Requirements
- Associate's degree in Nursing required; BSN or equivalent preferred
